Answers A and C should not be correct at all. According to the Prince2 book, 6th ed page 25, chapter 3.6, there are two type of products: management products and specialist products. No "major products" are defined in the entire book. There is no such thing called major products!
C is correct

PBP Step 2: Create the Product Breakdown Structure
A Project Product is broken down into the major products which in turn are broken down into further products to give a hierarchical overview. This is called a Product Breakdown Structure (PBS). A “mind map” diagram could also be used.
I dont think this is what is meant by hierachy. With hierachy its shown to to have several products, each product having its "sub products" (for example to set up a conference you need a venue, attendees, speakers etc.), and each sub products have their own sub items. So in that way its hierarchical. But being hierarchical doesnt mean there a certain order of products that need to be done first. This can be shown in a flow chart, based on the product breakdown (which products are interdependent, which need to be created first etc.).
